New Patient Registration

PLEASE BE AWARE THAT, CURRENTLY, OUR PATIENT LIST IS OFFICIALLY CLOSED TO NEW PATIENTS - WE ARE SORRY FOR THE INCONVENIENCE.

If you have your NHS Medical Card from your previous practice, please bring this with you when you register with the practice. It is not essential to have it to register. 

As a new patient you will be required to fill in two forms for each patient who is to be registering. These forms are obtainable from the receptionist and when completed, should include important information on current medication, immunisations, allergies and relevant medical family history. We also include a short questionnaire on alcohol consumption. 

Please note that to fully register with the surgery, you will need to complete a full registration form and provide 2 forms of ID.
